Naiyuan Qing b302164f29 feat(teams): per-team issue numbering cutover migration and resolver audit ...

- migration 132: re-backfill, counter sync, validation, NOT NULL, unique(team_id, number)
- drop legacy workspace-number uniqueness; teams now number independently from 1
- audit and convert remaining workspace-number lookups to team scope
- retire workspace issue counter from production write paths
