Replace hard-coded MiB byte conversions (e.g. `1024*1024`, `1<<20`, `1048576`) with the existing `_MiB` literal to improve readability and avoid repeating constants.
In the few spots where arithmetic involves signed values, the result is identical to the previous code assuming those quantities never turn negative.
Also switch to brace init on every declaration assigned from `_MiB`/`_GiB` literals so a future oversized value (e.g. `unsigned int x{4096_MiB}`) becomes a compile error through the C++11 narrowing check instead of silently truncating.
Extend unit tests to cover the 32-bit `size_t` overflow boundary and to assert equivalence for integer and floating-point conversions.

The indexes test call StartBackgroundSync(), which spawns a thread to run Sync(),
only for the test thread to wait for it to complete by calling IndexWaitSynced().
So, since the sync is performed synchronously, we can skip the extra thread creation
entirely and call Sync() directly.
Instead of having a single NodeContext::shutdown member that is used both to
request shutdowns and check if they have been requested, use separate members
for each. Benefits of this change:
1. Should make code a little clearer and easier to search because it is easier
to see which parts of code are triggering shutdowns and which parts are just
checking to see if they were triggered.
2. Makes it possible for init.cpp to specify additional code to run when a
shutdown is requested, like signalling the m_tip_block_cv condition variable.
Motivation for this change was to remove hacky NodeContext argument and
m_tip_block_cv access from the StopRPC function, so StopRPC can just be
concerned with RPC functionality, not other node functionality.
CTxDestination is really our internal representation of an address and
doesn't really have anything to do with standard script types, so move
them to their own file.
Calling interrupt_all() will immediately stop the scheduler, so it's
safe to invoke stop() beforehand, and this removes the reliance on boost
to interrupt serviceQueue().
This is safe because MilliSleep is never executed in a boost::thread,
the only type of thread that is interruptible.
* The RPC server uses std::thread
* The wallet is either executed in an RPC thread or the main thread
* bitcoin-cli, benchmarks and tests are only one thread (the main thread)

ed12d5df1b index: Fix for indexers skipping genesis block. (Jim Posen)
Pull request description:
This fixes a bug where indexers would skip processing of the genesis block. Preserves the current behavior of omitting genesis block transaction from the index.
